Worksheet Reporting Calendar
Diplomates can check their CM cycle end date and worksheet due date at My Account.
Anyone with a cycle end date of June 30 is on the one-time 54 month cycle UNLESS:
- The Diplomate has been granted a CM cycle extension, OR
- The Diplomate initially passed the exam in 2011 or thereafter.

1 Diplomates are normally on a 60 month reporting period that starts with the first CM cycle immediately after passing the exam. In 2011, some Diplomates were placed on a one-time 54 month CM cycle with proportionately reduced requirements.
2 CM credit can only be earned during this time period. If a Diplomate is granted a Cycle Extension, additional time will be allowed for earning CM credit and the Diplomate will receive new instructions.
3 Worksheets received or postmarked during this time period must include a $100 late fee.
4 Worksheets received or postmarked during this time period must include all audit records for all worksheet entries.
5 Worksheets submitted after this day likely will not be processed due to limited time before certificate expiration (decertification).
