CM Worksheet Audit Information

 
 

EXAMPLE OF AN AUDIT LETTER

If you are selected for an audit, the following letter will be sent to explain what documentation is required to substantiate your CM Worksheet. For general requirements about the point categories, you may read the ABIH CM Program Handbook or our CM FAQs.

EXAMPLE OF AN AUDIT LETTER
You have NOT yet been selected for audit.

Dear Diplomate;

The Board has asked us to express its appreciation for your submitted Certification Maintenance Worksheet and to inform you that your worksheet has been selected for audit. Please be assured that the audit selection was made on a random, not individual, basis. The audit is not intended to question your credibility but to provide a validation of the ABIH Certification Maintenance program.

You are requested now to provide documentation of your reported activities in the various CM categories. You may use the following as a guide in providing the information which the audit requires. The indicated information is requested. Please submit audit materials for a minimum of 40 CM points, 10 of which must be from Category 4/Attendance at Meetings and Educational Programs and from IH subject areas. If ABIH requires additional documentation we will contact you. To assist ABIH auditors, please submit your audit materials in chronological order by category. If a space is not checked the information which accompanied your worksheet is considered sufficient.

___ 1. Professional Practice - Your signed letter on your employer's letterhead and bearing your current organizational title will suffice. If you have had different employers during the CM cycle please list them with inclusive dates.
___ 2. Committee Service - Please provide a photocopy of the committee roster as published by the society for each year you served. If:
  • You served on a committee but your name was not published as a member, please explain. (A letter from the committee chair may suffice in this case.)
  • The committee roster was not published, please provide a copy of the committee membership roster, or a copy of pertinent documents or correspondence which identify you a committee member.
___ 3. Publication of Papers - If you published in a peer-reviewed journal, please provide a photocopy or reprint of the table of contents and the first page of the article. If you claim credit for a publication in a non-peer reviewed journal, send a copy of the article.
___ 4. Courses and Educational Programs - You are required to provide verification of attendance. Verification must include the following: a certificate, and/or letter, and/or attendance roster from the sponsor verifying the completion of your course or educational program. You must also provide hotel/motel receipts and/or approved travel expense vouchers for other than local programs.

Conferences and Meetings - local - attendance roster and/or certificate, and/or registration receipt, or the like; out of town - registration receipt as well as hotel/motel receipts and/or approved travel expense reports. (A photocopy of the badge is NOT sufficient.)

You should be able to provide evidence that your conferences and educational programs had CM points granted or be able to provide the needed information to obtain CM points.

The following courses and/or conferences are not in the ABIH database and have not been previously approved for CM points. You are required to submit course point inquiry forms and information on the following courses:








___ 5. Teaching - Proof may be copies of programs or sponsors' acknowledgments which show the length and topic of the presentation.
___ 6. Other Activities - If you have listed other activities on your worksheet, please provide essential support of your activity as outlined in various categories above.

In submitting the requested support of your CM activities, do not submit original materials which are essential to your personal records, as the Board will not ordinarily return them. Photocopies or duplicates of documents, receipts, etc., will be sufficient. Also, submit only the number and types of materials consistent with the guidelines presented above and your perception of what you would consider adequate if you were to review the report of another Diplomate.

All materials should be sent to our Lansing, MI office within 4 weeks. If questions arise during the audit, you will be contacted. If you have questions, do not hesitate to contact us. Your support of the ABIH Certification Maintenance program is appreciated.
