Seal/Stamp
Only Diplomates are allowed to order an embosser or ink stamp seal. Do this by:
1) Ordering and paying online. Diplomates will need to Log In >then proceed to the web page Make Payments & Purchases, or
2) Printing and mailing (or faxing ) the order form.

Replacement Certificates
Replacement certificates are available for $10 which includes S&H. To pay online, login and follow this webpath: Online Payment Forms > General Payment Form. Or you may pay via fax, mail or email using the Credit Card Payment Form. Note that the replacement certificate will likely not have the signatures as originally found on your certificate since the Board members may have changed.
